The Implant Built To Last

A new study was just published in the Journal of Unwanted Results, a peer-reviewed clinical publication by the International Implant Foundation in Munich, Germany.

The title says it all: Failure of All-on-4 Full-Arch Rehabilitation and Immediate Corrective Management Using the Technology of the Strategic Implant.

I want to share what this study found because you deserve to understand your dental treatment options.

What the study looked at

Four patients between the ages of 50 and 76 arrived at a clinic with failed full-arch dental restorations.

All four had previously received All-on-4 implants, the widely marketed approach that replaces an entire arch of teeth using only four supporting implants.

In every case, the restoration had failed within three to twelve months of placement.

Why All-on-4 failed in these cases

When you chew, approximately 90 percent of the biting force is concentrated in the back of the jaw.

The All-on-4 system places its four implants predominantly in the front of the jaw, creating a fundamental mismatch between where the force is generated and where the support actually is.

The study describes this as a single point of failure problem. Because the entire arch depends on only four implants, if any one becomes unstable or gets placed under unequal load, the whole system is at risk.

Think of a table with four legs all placed at one end.

In the four cases documented, complications included severe bone loss around implant sites.

One patient lost most of her implants within six months. A former professional boxer lost all implants on the right side of his upper jaw entirely. All four patients reported not just physical consequences but psychological and financial ones as well.

What actually worked

In each case, the failed system was removed and immediately replaced using Corticobasal implant technology, specifically the Strategic Implant system. Instead of four implants, eight to ten were placed per arch depending on the complexity of the case.

The key difference is that Corticobasal implants anchor directly into the dense cortical and basal bone layers deeper in the jaw, bypassing the softer surface bone that had already been compromised.

This bone is far more resistant to shrinkage over time and does not depend on having a large volume of surface bone to begin with.

The wider distribution of support also means chewing forces are spread evenly across the full arch, including the back where those forces are actually concentrated. There is no single point of failure.

All four corrected cases remained clinically stable at follow-up periods of one to two years. Every patient reported improved comfort, function, and confidence.

What this means at Asenjo One Visit Dentistry

This study confirms what we see regularly in our clinic.

We use Corticobasal implant technology because the evidence supports it. More support points. Anchorage in denser, more stable bone. Immediate loading that works with your jaw's biology. And a methodology validated through peer-reviewed research across thousands of patients worldwide.

The study also notes that peri-implantitis, the infection around implants that causes bone loss and is one of the leading reasons traditional implants fail over time, is documented to not occur with Corticobasal implants in immediate functional loading protocols. That distinction alone changes the long-term picture significantly.

Choosing a full-arch restoration is one of the most important health decisions you will make. The method matters. The materials matter. And the number and placement of implants matter more than most people are told before they sign a treatment plan.
